Membership Information and Terms

Membership Services - Members are entitled to their choice of a one hour Swedish massage or Reflexology Intro or signature facial once each month. Any service enhancement requests will be charged separately and additional to membership costs.

Rollover Privileges - Members may rollover any unused services to the following month. Upon termination of your month to month membership, accumulated member service packages expire and are non-refundable 90 days after you cancel your membership, accumulated member services expire 90 days after the last day of a Prepaid Term Membership.

Retail Discounts - A 10% discount is given to all members on retail product purchases.

Guest Privileges - Members may bring in an unlimited number of guests and the guest may pay member prices for services.

Membership Freeze Option - A membership may be frozen for a minimum of one month and a maximum of three months.

Memberships can only be frozen two times in a 12 month period for a maximum of 3 months at a time. A membership must be unfrozen a minimum of 30 days before it can be frozen again. A "freeze fee" may apply.

Members can redeem accumulated services during a freeze but cannot utilize any additional membership benefits (i.e. bringing guests, retail discounts).

A minimum of one EFT payment must have been made before a membership is eligible to be frozen. ( You must provide a 30 day written notice to Hoya Wellness Spa before the freeze period begins. )

Assignability - Memberships are non-transferable and non-refundable.

Misc. Fees and Costs - Hoya Wellness Spa is in no way, under any circumstances, responsible or obligated to pay for member’s insufficient fund penalties, returned checks, and/or over the limit fees.

A $30 fee will be charged for all insufficient funds and returned checks. 

Notification of Changes - Hoya Wellness Spa must be notified about any changes in address, contact information and/or credit card information within fifteen (15) days of such change.

Spa Rules - Guests must follow spa rules and regulations. Hoya Wellness Spa reserves the right to refuse or discontinue services for any reason at any time.

Hoya Wellness Spa reserves the right to modify/change spa rules, regulations, services and pricing in its sole discretion with reasonable notice.

Appointment Cancellation - 24 Hour advance notice is required on any appointment cancellation. Any cancellation with less than 24 hours of notice are subject to a cancellation fee amounting to the cost of the scheduled service. Client who miss their appointments without giving any prior notification will  be charged full for the scheduled services. 

Accessibility and Minors - No person will be denied admission based upon race, religion or appearance. CHILDREN UNDER THE AGE OF 12 SHALL BE STRICTLY PROHIBITED FROM ENTERING THE SPA. No children under the age of 18 will be able to receive any service without the express written consent of their parent or legal guardian. For children 15 and under (or where required by state law), a parent or legal guardian MUST remain in the treatment room and for all minors ages 16 or 17 the parent must remain on premises during services.

Membership Monthly Renewals and Termination - All memberships paid monthly will renew automatically  month to month.   For cancellations, holds, and/or changes to memberships for any reason, members must notify Hoya Wellness Spa via email at hoyaspa@gmail.com at least 30 days prior to next auto-draft for membership dues, putting “membership cancellation or membership hold” on the subject heading of email.  Alternatively, members may cancel membership in person and in writing at Hoya Wellness Spa.   In turn, Hoya Wellness Spa will send a confirmation via email after the changes have been made to the account or provide a termination confirmation to member who cancel in person. Hoya Wellness Spa may take up to 5 business days to respond by e-mail.

The termination of a membership shall begin 30 days from the date the written termination request is provided to Hoya Wellness Spa (the “30 Day Period”) and can not be during the first 3 months of membership.   Any and all scheduled charges during the 30 Day Period, including, without limitation, monthly membership dues, shall still be due and owing and processed by Hoya Wellness Spa, and such charges shall not be prorrated.  For example,   if a member’s membership renews on the 14th of each month and on  April 1st a written notice to cancel is received by Hoya Wellness Spa,  April 14th's monthly membership due will be due and charged and the member’s last day to attend and enjoy membership benefits shall be May 13 of the same year.

General Provisions - This Agreement supersedes and replaces all prior undertakings, communications, agreements and understandings, oral or written, between the parties with respect to the subject matter hereof.  This Agreement constitutes the entire agreement between the parties with respect to the subject matter hereof.  No change, modification, termination, or attempted waiver of any of the provisions of this Agreement shall be binding upon the Hoya Wellness Spa unless reduced to writing and signed by Hoya Wellness Spa.  Paragraph headings throughout this Agreement are solely for the convenience of the parties and are intended to have no legal meaning in and of themselves.
